Vestibular schwannomas, also known as acoustic neuromas, are relatively common tumours that arise from the vestibulocochlear nerve (CN VIII) and represent ~80% of cerebellopontine angle (CPA) masses. Bilateral vestibular schwannomas are strongly suggestive of neurofibromatosis type 2 (NF2). Vestibular schwannomas are best evaluated with gadolinium-enhanced T1-weighted MRI, which can detect tumors as small as 2–3 mm. Recent studies have found that the reported sensitivity and specificity of noncontrast MRI is almost equivalent to that of gadolinium-enhanced T1-weighted MRI. The appearance of the vestibular schwannoma on the MRI is a rounded, enhancing mass with extra-canalicular (outside the internal auditory canal), intra-canalicular, or both components.
